wabble on the DVR spindle unless I used the set screw. During the course of
turning it would loosen and I would have to tighten it again. What gives?

I can think of a couple of possible causes. The DVR spindle diameter may be
a tad smaller than the Grizzly. The other possibility is that the flat end
of the chuck thread insert may not be seating against shoulder on the DVR
spindle. (or both.)

I took it off and removed the insert. After pulling the set screws out
it was difficult to remove from the spindle. This made me worry that I
had cross threaded it but when I examined all the threads I could see no
damage.
I examined the threads on the chuck after pulling the insert and they
looked OK. I examined the inner threads on the insert and they looked
OK. However the outer insert threads had a damaged bit that probably
kept me from screwing it all the way into the chuck. I got out a file
and worked on the knick in the thread until I could***the insert all
the way into the chuck without much effort. The damaged part of the
threads is at the end of the threads and not the beginning. I'll have
to look at it again this weekend to see if the set***could have
damaged the threads.
