Does someone please have an experience or a lead to a facility that could scan a small object and 3D print it from a thermoplastic material?  Thanks.

Try looking for a hacker group in your town.  There are groups like these
all over the country that provide space and tools (for a membership fee)
to individuals interested in building things.  The group in my town has a
3D printer but I don't know about a scanner, but it isn't close to you so
it isn't relevant to your need.  Just for information, they also have a
lathe, a mill, welding equipment, woodworking tools, and they give
classes in how to use them and in other subjects like computer
programming.  I found out about them when I overheard a conversation at
my local electronic parts supplier.
